Service Description

Physical exams alone are not covered by insurance. Please bring any forms, prior medical records, medication bottles with you. For work physicals bring in form from employer. Also indicate if a TB test is necessary. For school physicals please bring current vaccine records. For sports physicals, if recent injury/surgery must bring documentation. Please complete the forms that you will receive upon booking in your email. Arrive at least 15 minutes early to complete any paperwork if not completed online.
