Service Description

Physical exams alone are not covered by insurance. Please bring any forms, prior medical records, medication bottles with you. For DOT physicals please bring any pertinent medical records from your PCP and be prepared to complete from required from FMSCA. Please complete the forms that you will receive upon booking in your email. Arrive at least 15 minutes early to complete any paperwork if not completed online.
